A one-time, non-refundable registration fee of $50 per camper will be billed to FACTS upon registration completion.

As parent/guardian of the above-named student, I give RSM Christian School Elementary Summer Camp my permission to administer to my child(ren) the allergy/anaphylaxis treatment/medication that we have maintained/supplied when needed and as I have instructed on the FARE Care form.

My child(ren) has/have permission to participate in all RSM Christian School Elementary Summer Camp activities. If my child(ren) become(s) ill or sustains an injury while in the care or under the supervision of Rancho Santa Margarita Christian School or any of its representatives, they are permitted to administer first aid for my child(ren)'s relief. If it is not practical to return my child(ren) to us or to receive our instructions for my child(ren)'s care, consent is hereby given to admit my child(ren) to any hospital; consent is also given to any licensed physician or surgeon called, or to whom my child(ren) is/are taken, to administer such treatment, drugs, and medicines, and to perform such procedures as he/she shall think the existing emergency requires for the relief of pain and to preserve my child(ren)'s life and health. Authorization is also given for such other measures or procedures as may be required. I hereby agree to reimburse Rancho Santa Margarita Christian School or any of its representatives for any expenses incurred in the care of my child(ren) should any type of medical treatment be necessary. This would include hospitals, doctors, ambulances, etc…

For each child who carries an Epi-pen, please download, fill out, and return a FARE Care Plan form.

You are consenting to a one-time Registration Fee of $50 per camper through your FACTS account.

All camp weeks selected will be charged regardless of attendance. We are unable to accept cancellations after registration.
